Assessment Tasks

Copy and paste from the following data to produce each assessment task. Write these in plain English and spell out how, when and where the task is to be carried out, under what conditions, and what resources are needed. Include guidelines about how well the candidate has to perform a task for it to be judged satisfactory.
Copy and paste from the following performance criteria to create an observation checklist for each task. When you have finished writing your assessment tool every one of these must have been addressed, preferably several times in a variety of contexts. To ensure this occurs download the assessment matrix for the unit; enter each assessment task as a column header and place check marks against each performance criteria that task addresses.

Identify factors and barriers that contribute to a complex RTW case 
Identify and analyse the impact and interplay of injuries, pre-existing conditions and secondary conditions on the case 
Identify mechanisms to support injured persons available within the organisation 
Determine situations where early intervention or changes to RTW management are required 
Determine a support strategy for particular issues and complexities 
Identify needs of the injured person and indicators of complex issues and risks 
Seek advice from external specialists where required to assist in development of a sustainable RTW plan for injured person 
Identify suitable duties, reasonable adjustments and area of work for injured person based on identified needs and advice from specialists as required 
Determine a suitable RTW program for the injured person by liaising with the required authorities 
Document an RTW plan identifying key duties, requirements, timeframes and expected outcomes 
Communicate RTW plan to stakeholders according to health privacy principles 
Manage RTW plan according to organisational policies and procedures 
Liaise with team leader and injured person to monitor progress towards outcomes 
Identify and manage barriers, emerging conflicts and issues relating to the RTW plan 
Communicate with injured person to minimise confusion and concerns 
Review early intervention strategy and make changes as required 
Develop solutions and options for future action to attain outcomes with the injured person, required authorities and stakeholders 
Escalate issues that cannot be resolved within own work role to required personnel
